Request for Proposals - Medical Insurance of Staff

Nabil Bank Ltd. invites sealed proposals from interested firms/companies for insurance services for Medical Insurance of its 2300 staffs.

Required Coverage
1. Coverage for Employee, Spouse and Dependent Children.

2. Consultation Fee up to a maximum of NPR. 1,000.00 per visit.
3. Room, Board and Nursing Expenses, ICU Expenses, Surgeon, Anesthetist, Medical Practitioner, Consultant, Specialist Fees.
4. Blood, Oxygen, Anesthesia, Operation Theatre, Surgical Appliances & Fitting, Diagnostic Material, Injection Cost and Lab Test Charges.
5. Other Investigation Charges' like USG/ECG/Scan, EEG, MRI, Electrical/ Massage Treatment, Endoscopy, Homeopathy treatment for special cases such as Jaundice.
6. Physiotherapy Charges, Lenses for Cataract Operation, Transplant of Body Organ.
7. Maternity Benefit with maximum limit of RS. 90,000.00 per year.
8. Dental treatment and Optical treatment.
9. Recurring type of diseases (T.B., Ulcer, Diabetes, Hypertension, CAL Paralysis, Thyroid, etc.) Epilepsy, 

10. Evacuation Benefits Coverage up to NPR. 5 Lakhs in case of rescue/ Lakhs in
evacuation due to severe illness and during policy period.
 

Terms and Conditions

1. Bidders must submit OLPA occidens following documents. 
a) PAN/VAT Certificate
b) Audited Balance Sheet of Last Two years
c) List of Prime Customers
d) Firm/Company Registration (renewed) e) Copy of tax file return (updated)
f) Company profile with brief introduction of promoters and key management team 

2. The sealed quotation should be submitted in an envelope addressing Administration Department, Nabil Bank Ltd, with name of the bidder's firm and address. Sealed quotation should reach to the Bank's Head Office, Administration Department, Ghantaghar Building 4th floor, Kathmandu, Nepal, not later than 1:00 PM, 29 December 2023 (Poush 13, 2080). Bid documents received after this deadline will not be accepted.
3. The bid shall be opened on 11:00 AM, 1st January, 2024 (Poush 16, 2080) at Bank's Head Office, Administration Department, Ghantaghar Building, Kathmandu, Nepal,
4. In case the last day of bid submission/ opening falls on a government holiday, then the next working day shall be applicable for the same.
5. The bank reserves the right to accept or reject in part or whole of any sealed quotation without assigning any reason whatsoever and will not be compelled to accept the lowest bid.
